Initial
Appointment
Please arrive at least 15 minutes prior to your first
scheduled appointment. To assist our staff, we ask
that all new patient forms (below)
and health questionnaires be completed prior to your
scheduled appointment time.
Appointments
Patients are seen by appointment only. Please call
our office, in advance, so we may schedule you at
times that are most convenient for you. We also request
that you provide at least 24 hours notice for all
cancellations. We keep a wait list and would like
to offer available appointments to other patients
as far in advance as possible. A $25 fee will be charged
for all scheduled appointment no-shows, or any failure
to cancel an appointment within the 24 hour time period.
Insurance
We offer a valuable service to our patients - free
of charge - we file your insurance claims for you.
We also accept assignment on many, but not all, insurance
plans. This means we bill your insurance company,
they pay us directly their portion of the fee, and
we only collect the patient’s portion from the
patient. What makes this service sometimes very difficult
is the fact that there are over 23,000 different insurance
plans in the United States. It is not possible for
us to know everything about these plans. It is ultimately
the patient’s responsibility to know their plan’s
requirements. We participate in the following insurance

We
will gladly verify any out of network benefits
for you if we do not participate with your insurance
plan. Care Credit, similar to a medical credit card, is also available at our offices to assist you in your payment options: www.carecredit.com.
Automobile and Workers Compensation Claims
Our office follows your health insurance plan’s
rules. If your insurance requires a referral
from your primary care physician, please bring one
with you on your first visit. We will preauthorize
your visits if it is required by your plan. We will
not submit to your health insurance plan until your
PIP is exhausted or your worker’s compensation
claim is denied. In order to streamline the process of obtaining authorization for your workers compensation claim, we require your adjustor to phone us so that we may obtain all necessary information before your physical therapy treatments begin.
Auto
Claims Payment
Patients using their automobile’s PIP (Personal
Injury Protection) are required to pay at the time
of service. We will supply all of the documentation
necessary for you to submit to your insurance plan.
Once you receive a PIP exhaustion letter, we will
begin to submit claims to your health insurance plan.
At this time, you will be required to pay any necessary
co-payments and deductibles.
